I am looking for a good travel form that which could be used for gathering all the information needed when booking a trip and also another good form that can be used once the travel arrangements have been made which would look really impresive...
Janet, I work for a small corporation with 60 employees and I just keep it simple. The listing of necessary documentation is: name, function attending, others attending with this person, hotel accomodations, flight arrangements and car rental. I attach a copy of the flight arrangements to the sheet which enables me to get my hands on the info quickly if necessary. Judie Petersen Western States Learning Corporation

I also have a travel itinerary form that I have created for all traveling that includes flight information, limo, hotel, car rental and scheduled meetings. I could forward you a copy if you forward me your e-mail address at frankl@mutualtrust.com
Also, I would be interested in seeing the other forms that you received from others ... can you forward me a copy of those?

I have a quick, easy spreadsheet I use for my boss's itinerary. It's color coded for flight, rental car, and hotel and lists addresses, phone numbers, and confirmation numbers. I give him a copy to take on his trip and one to give to his wife. If you're interested, send me an email (carintener@aol.com) and I'll forward it to you.

Travel itinerary templates are available for downloading via
www.Office.Microsoft.com
Take a few of the examples and recreate your own via Word, or Excel if so
desired.

I have a document that I email out to travellers once all the arrangements have been made, that details out their confirmation numbers, flight details, etc.

I use twoforms. The first one is the template I use/need for the first time traveler. The second one is the itinerary I complete and give to traveler for their spouse/family. If you would be interested in receiving these, please email me at amrheinj@bcbsmo.com and I will send them to you.

You can find free downloadable templates from Microsoft. Here is a link and the web address. I use these templates often and they seem to work out well. Once you save the template, you can edit as you wish to fit the need.
